Ok, so your device does have a valid address and the network address translation is being performed on that device.

At this point, I think we're looking at a problem with that Netgear router. Your forwarding is correct, but it does not appear that traffic is being forwarded.

Ensure that you don't have the built-in OS X firewall turned on. If you don't, then I'd go ahead and upgrade the firmware on that router. If that doesn't work, I'd contact Netgear support.
